I agree to abide by all the Rules, Regulations and Procedures of “The Barkskies” Furcare which are in effect at this time, and which may be amended or updated at anytime, without referral back to myself. I further agree to be bound by “The Barkskies” Furcare Disclaimer written within, which I have read and understood in full. I certify that the dog/s who's details I have entered on this form, is/are fit and well and has NO Medical conditions that may affect them whilst swimming, boarding, daycare and may stop this dog/s from taking part and is NOT a Hazard to any persons, our Team or other dogs with in or around the centre. I agree to take FULL RESPONSIBILITY for this/these dog/s and any of thier actions. I further understand that I will be fully responsible for my own dog/s whilst in and around the premises and whilst in the pool and will not hold “The Barkskies” Furcare or their staff or any other person responsible for my dog/s at any time - the dog/s is my sole responsibility, as I have chosen to swim/ board my dog at these facilities named above, I hereby release and discharge “The Barkskies” Furcare, their Staff and/or their representatives not to sue from ALL liability to the above person (myself) and dog/s , for any and all loss or damage and any claim or demands therefor on account of injury to the person, dog/s or property or resulting in death of the undersigned or dog/s relating to taking part, whether caused by the negligence of the releases or otherwise and hereby agree to indemnify and save and hold harmless the releases for any risk of bodily injury, death or property damage arising out of or related to this activity, both for myself and my dog/s Should I feel that the setting, equipment, “The Barkskies” Furcare, their staff or representative is unsafe, I will refuse to sign or take part, I also agree further visits to “The Barkskies” Furcare, help from their staff or representatives will be covered by this document that I have electronically signed.
